Eligibility:

• Macomb County residents
• Ages 19-64
• U.S. citizens
• Not disabled
• Not qualified for Medicaid or other medical programs
• Income less than 35 percent of federal poverty level (about $200-$250 per month)

(Eligibility determined by Michigan Department of Human Services)

Eligibility:

• Macomb County residents
• Ages 19-64
• Adults with no minor dependent children
• Individual and spouse income 100 percent of federal poverty level, or less (about $800 per month for individual, $1,070 per month for individual and spouse)
• Not eligible for any other medical coverage (Medicare, Medicaid, retiree or any other commercial medical benefit)

Information required for enrollment:

• Completed application
• Current photo identification
• Medicaid rejection letter from Michigan Department of Human Services
• Proof of current address
• Copy of Social Security card
• Proof of individual and spouse income
• Bank statement and mortgage or lease statement
• Completed income/expenses form

Coverage starts at the beginning of the next month after approval of application. Once approved, members receive ID card and member handbook.
