Annual Summary Financial Report & Club Inventory Report

These forms must be completed by the club treasurer and general leader at the end of each program year.  Both forms must be turned in to the MSU Extension 4-H Office no later than November 1.  A copy of each form should be kept for club records.

Club Registration Forms
Club registration begins September 1.  Forms for new and returning 4-H members and leaders will be given to the club general leaders.  Leaders with no club affiliation receive their enrollment forms directly.  IMPORTANT:  Registrations are accepted at any time, however, if a returning 4-H member wishes to show projects at the Armada Fair, he or she must be registered with the 4-H office by 5 p.m. by the 1st business day in November.  New members planning to show projects at the Armada Fair must be registered by 5 p.m.by the first business day in April.  Members submitting registrations after the deadline must request and be granted special permission from the Macomb County 4-H Council before they can show projects at the Armada Fair.

NEW!  Online Registration

General leaders should contact the Macomb County 4-H Office at macomb.4h@macombcountymi.gov or (586) 469-6431 if their club plans to register online for the 2011-12 program year.  Open IDs will be assigned to returning members and leaders of the group. New members will be assigned an Open ID when they enroll.  Please note that all volunteers must complete the Volunteer Screening Process before enrolling.

Once returning members and leaders receive their Open IDs and passwords, they may enroll online at https://enrollment2.4-h.org/login. New members enroll online at https://enrollment2.4-h.org/login without an OpenID or password.

General leaders are still required to turn in the following forms every year:

2.  Club Information Sheet

3.  Club Bylaws

4.  Code of Conduct form for each volunteer in your club

By registering online, members agree to follow the 4-H Code of Conduct for Members and Parents/Guardians/Relatives.  Failure to comply with the Code of Conduct can result in loss of privileges normally accorded to 4-H members.

Livestock Sale Participation Form

All youth who plan on doing any market livestock project at the 2012 Armada Fair must return the completed form to MSUE Livestock Sale Committee Secretary, Jill Senyk at 13678 Mesmer, Allenton MI, 48002 by the due date of January 13, 2012.

Member Information Form

Use this form to register as a 4-H member in Macomb County.  By signing this form, members and parents are agreeing to abide by the 4-H Code of Conduct.

Funding for MSU Extension is based, in part, on the programs and activities in which our staff, volunteers and youth participate.  Clubs and committees should use this form as a sign-in sheet for each workshop or training your club or committee does that is educational in nature and falls within a project area.  Include events such as the 4-H Chinese Art Exchange, Macomb County 4-H Craft Workshop, K-9 Karnival, animal clinics, meetings with educational components, etc.  Return completed forms to MSU Extension - Macomb County, Attn:  4-H, 21885 Dunham Rd., Suite 12, Clinton Township, MI  48036

Treasurer's Report Form
This form is to be submitted to the Macomb County MSU Extension 4-H Office with the Secretary's Report and minutes after each club meeting.

Volunteer Enrollment Forms

Once the Volunteer Screening Process (VSP) has been completed, volunteers must submit completed 4-H Volunteer Enrollment and 4-H Volunteer Code of Conduct forms yearly.  Forms should be mailed to Macomb County MSU Extension, Attn: 4-H, 21885 Dunham Road, Suite 12, Clinton Township, MI  48036 or faxed to (586) 469-6948, Attn: 4-H.

Volunteer Selection Process

The Volunteer Selection Process consists of an application, interview, criminal record check, sexual registry check, three reference checks and an orientation. New volunteer orientations are typically held monthly. Please see the calendar for dates. Interested applicants please contact the MSUE Extension office at 586-469-6431 to register for the next orientation.

Adults interested in volunteering for 4-H programs need to complete the attached forms and mail to: Macomb MSU Extension 4-H, Volunteer Selection Process, 21885 Dunham Suite 12, Clinton Township, MI   48036 or bring to the new volunteer orientation.

Workshop Registration
Workshop Registration forms must be completed and returned to the Macomb County MSU Extension 4-H Office along with any applicable workshop fees if you plan on going to a workshop at Kettunen Center.  You will be required to take a completed Media/Medical Release form with you to the workshop.
