Flu Vaccine Registration and HIPAA Forms

Save time in line!  Please print and complete a registration form for each individual who is to receive a flu vaccination.  Child forms must be completed and signed by a parent or legal guardian.

Season flu vaccine cost is $15.00, payable by cash or check.  Children under the age of 19 years who do not have insurance coverage or whose insurance does not cover the cost of flu vaccine can enroll in our Vaccines for Children program and obtain the flu vaccine for the administration cost of $10.00.

If your child has MICHILD coverage, please contact the phone number on the back of your MICHILD Health Card to obtain the name of a physician in the area who will provide the flu vaccine to your child, at no cost to you.  We do not contract with MICHILD and your cost would be $15.00 at our clinics.

We do accept eligible Medicaid coverage for children and adults - we will bill Medicaid and there will be no out of pocket cost to you.
